Requirements to avail the "Libreng Palupa" Program

• Referral from the Mayor
• Voter’s certification
• One (1) valid ID (photocopy only)
• 2x2 ID picture
• Cedula for the current year (photocopy only)
• Certificate of No Ownership of Real Property within the barangay

To be eligible to the Iskolar ng Bayan Program, the applicant:

• Must be a Filipino citizen;
• Has graduated from any public HS in the country within 2 years prior to S.Y. 2019;
• Must provide a certification from the public HS, duly signed by the school principal, of the rank of the applicant in the Top Ten of the graduating (Grade 12) class;
• Must be enrolling for the first time in college and has no credited college subjects;  
  If applying through automatic admission (i.e. did not take the UPCAT, but satisfies all other eligibility requirements), the applicant must:* Apply to a UP campus that is within the region of his/her high school [see Region Assignment in Table 1]
  * Submit a certified True Copy of Grades/Transcript of Records from which his/her GWA can be computed for purposes of ranking in lieu of the UPG;

• Personal letter addressed to the Municipal Mayor
• Referral from the Mayor
• Birth certificate of the student
• CEDULA (student and client)
• Voter’s certification
• One (1) valid ID (student and client)
• Barangay Clearance
• Barangay Indigency
• Barangay Waste Segregation
• Photocopy of Grades for the last semester
• Registration form

Hospitalization Assistance Requirements (Client and Patient)

• Referral from the Mayor
• Cedula for the current year (photocopy only)

• Copy of Billing from the hospital;

Anti-Rabies Vaccine Assistance Requirements (Client and Patient)
